Peace of Mind Transitions

Step 5 · Put your plan into motion

Coordinated Action

Bring the right people together, with clear responsibilities and a shared path forward.

A plan becomes useful when the next steps happen

Once Your Family Plan or a standalone engagement sets the direction, we help coordinate the people, information and practical details needed to carry it out. Your family has a familiar POM contact to keep the agreed work connected.

Some actions can proceed together; others depend on legal advice, tax information, a property decision or a family approval. We help make that sequence clear.

Experts and professional partners

The right expertise for each part of the plan

01

Legal expertise

Lawyers provide advice and drafting for wills, powers of attorney, trusts, estate matters and property transactions. POM helps organize the information, questions and follow-up so their work connects with your family plan.

02

Real estate expertise

Licensed real estate professionals advise on property value, buying or selling, market timing and transactions. We coordinate property preparation and timing with your housing choices, cashflow needs and family priorities.

03

Accounting and tax expertise

Accountants and appropriately authorized tax professionals address returns, estate and trust filings, business interests and complex tax questions. POM coordinates records, responsibilities and deadlines with the planning work.

04

Home, care and transition support

Renovation, accessibility, decluttering, moving and care providers help carry out the practical work. We organize the sequence, quotations, approvals and family communications within the agreed coordination scope.

Home projects and transitions

Residence One

Our discussions with Jay have focused on how Residence One could support home renovations and smaller projects arising from a family’s housing plan.

Where appropriate, we can explore a separately scoped project with Residence One—whether you are adapting your current home or preparing for a transition. The project scope, suitability, availability and quotation are confirmed with the provider before you commit.

The family plan sets the direction

Home & Lifestyle Planning helps you compare staying home, modifying the property, additional care and a move. Construction or property work follows the agreed decision and budget.

Residence One’s work is separately contracted and priced. Any POM project coordination is also defined in your engagement.

How coordination works

Clear owners. Agreed timing. Follow-through.

01

Agree on the actions

Turn the written plan into a list of tasks, owners, target dates and decisions. Confirm which work POM coordinates and which belongs with your family or a professional.

02

Connect the right people

With your permission, connect the relevant experts and share the information needed for their work. We can coordinate with your existing professionals or help identify suitable providers.

03

Confirm scope and costs

Obtain the relevant proposals and quotations. You approve the providers, fees and work before it begins; specialist and implementation work has its own terms.

04

Follow through together

Track progress, decisions and dependencies, keep the family informed and flag changes needing approval. Record completed work and remaining actions for the next review.

Support during caregiving, incapacity and loss

A change can bring unfamiliar decisions and practical tasks. Within an agreed engagement, POM can help organize priorities, identify who has authority, coordinate professional contacts and keep authorized family members informed.

Care providers deliver personal and clinical care. Counselling and grief therapy belong with qualified specialists. POM coordination does not replace emergency services, clinical care or the authority of an appointed decision-maker.

You remain informed and in control

You choose the professionals and approve the work. We coordinate with your existing lawyer, accountant, real estate professional or other provider, and help connect additional expertise where needed.

Each professional remains responsible for their own advice and services. We confirm information-sharing permissions, provider roles and any relevant referral or compensation arrangements before proceeding.

Know what the coordination includes

Your proposal defines POM’s actions, meetings, support allowance, reporting and fees. Legal, accounting, real estate, construction, moving and other third-party services are separately scoped and priced unless expressly included in writing.

We record completed work, outstanding decisions and the next review date. Additional paid work requires approval; estate administration and ongoing executor support require their own engagement.
